Requirements
Applicants must prepare and submit the required documents for each stage of the scholarship process.
Upon Application
Applicants must submit the following:
- Duly accomplished Online Application Form
- High School Grade 7 to 12 Transcript of Records
- Proof of Financial Status
- Proof of Admission or Acceptance Letter from any of the eight partner schools
- Birth Certificate of the applicant
- Portrait Photo of the applicant
The High School Transcript of Records must include the final General Weighted Average.
If 2026 grades are not yet available, Grade 12 grades may be up to December 2025.
For proof of financial status, applicants may submit one of the following, depending on the parent or guardian’s employment status:
- Latest Income Tax Return, if employed
- Certificate of Indigency, if unemployed
- Copy of OFW Employment Contract with Compensation, if working abroad
Upon Admission to the Scholarship Program
Once admitted to the scholarship program, applicants must submit the following:
- Certificate of Enrollment or Registration Card for AY 2026–2027
- Copy of School Calendar for AY 2026–2027
- Copy of School Grading System, if the school does not use the percentage grading system
- Copy of Course or Program Curriculum
- Copy of Parent or Guardian’s Government-issued ID for notary purposes

Terms and Conditions
