- Certificate of Indigency signed by the Punong Barangay where the applicant resides. (Can be Obtained from the Barangay Office)
- School ID
- Two photocopies of a current and valid School ID (front and back).
- Photograph
- One piece of 1×1 photo. (Must be Latest)
For Continuing Beneficiaries
- Proof of Enrollment
- Certified True Copy or electronically generated Certificate of Enrollment (COE) or Certificate of Registration (COR) showing the number of units enrolled. (Can be Obtained from the school admission office or School Student Portal)
- Leave of Absence (LOA)

- Latest Income Tax Return (ITR): Short detail for you to have an idea of what it is: ts total income and taxes paid for the year. (Can be Obtained from the BIR Office)
- Bureau of Internal Revenue (BIR) Form 2316: Short detail for you to have an idea of what it is: Summarizes income earned and taxes withheld by an employer. Provides evidence of income for employees. (Can be Obtained from the BIR Office or Principal Employer)
- For Unemployed Parents/Guardians
- Certificate of Tax Exemption from the BIR: Short detail for you to have an idea of what it is: Proof that the parent or guardian is not required to pay income tax due to lack of income. (Can be Obtained from the BIR Office)
- Certificate of Indigency
- Certificate of Indigency from the Barangay. (Can be Obtained from the Applicants Barangay Office)
- Additional Indigency Documents

- Payment via CHEDROs

- Enrolled in Private HEIs or SUCs/LUCs: Institutions with COPC outside the region.
- Less than Ten Grantees: Programs at Private HEIs or SUCs/LUCs with fewer than ten grantees.
- Unresolved Issues: Institutions with unliquidated balances or verified complaints related to Student Financial Assistance Programs (SFAPs).

- Enrollment Issues: Not enrolling within the designated period.
- Program Compliance: Not enrolling in CHED-recognized programs at Private HEIs or SUCs/LUCs with COPC.

Hi po, ako po si Tabilangon Jeanella Faye D. Nag-aaral sa Mabalacat City College, 1st year student. Pinapa-aral ko po ang aking sarili at tinutulungan nalang po ako ng aking ate na may pamilya narin sa mga gastusin sa school. Ang aking mga magulang naman po ay hindi po nila kaya na mapag aral kami sa college dahil narin po siguro na may tatlo pa akong kapatid na nag aaral, yung dalawa po ay nasa elementary at yung isa po ay nasa high school. Kaya naman po gumagawa nalang ako ng paraan para makapag tapos ng college at kasalukuyan po akong nasa bahay ng tita ko at duon po ako nag tatrabaho. Kaya ako po ngayon ay humihingi ng tulong sa pamamagitan ng pag sali ng scholarship sa CHED.
